Registered nurses (RNs), licensed practical nurses (LPNs), and state-tested nursing assistants staff our Lebanon care teams. They manage, observe and evaluate your care on a routine basis.

Our memory support environment is safe and secure. But also comfortable and homey. Meaningful activities abound, geared to the interests and strengths of each resident.

Transitional Care

Seamless care from your very own Otterbein Lebanon team — on campus — optimizes your recovery. We manage your condition until you can return to your daily routine.

Transitional care is by definition short term — usually less than 30 days. But those few weeks are crucial. Skilled medical care is vital. So is continuity of communication and coordination of care. That’s why your transition is managed by people you know at Otterbein.

The Transitional Care Unit (TCU) has 24 beds, 16 private and eight semi-private. The TCU is staffed with skilled nurses and therapists who work in conjunction with your hospital discharge department or home health agency.

As you progress, we teach you to manage your own needs. You learn to understand medications. You’ll recognize the signs and symptoms that should lead you to contact a health provider. You return home stronger and wiser.

Assisted Living

Assisted living is different for everyone. The goal is to keep you enjoying your life in comfort, as independently as possible.

Assisted living basically means that you will receive regularly scheduled help with some of life’s daily activities. Like meals, housekeeping and transportation. Our health care professionals work with you to customize a plan.

Together, you’ll determine how much or how little assistance you need day to day. Services range from minimal support to concentrated services for specialized needs. We’ll review your plan regularly and adjust as needed.

You can be part of the Otterbein Lebanon family without living on campus. A high level of personalized care and therapy is available within our nine-county service region.

When you’re recovering from surgery, illness, injury, a wound or a fall, there’s a special Otterbein Home Health team ready to help. We’ll bring the best personal and professional care possible to your doorstep. 

Skilled nurses, home health aides, and physical, occupational and speech therapists come to you. We take the time to listen to your concerns, guide you, and share encouragement.

Since physical healing benefits from spirituality, spiritual caregivers will pray, read scripture, or simply chat with you as you wish. We believe you deserve to be comfortable, happy, safe and respected.

Adult Day Services

Otterbein’s Adult Day Services at Lebanon is a joyful place. Recreational and therapeutic programs fill your loved one’s mind, body and spirit with life.

Here, your loved one is safe, supervised by a caring staff with 40 years’ experience. Individual and group activities increase self-esteem and enjoyment. Exercise, group crossword puzzles, music and arts, crafts and cooking, games, parties and spiritual programs happen daily.

We can also help your family member access a wide range of health and wellness services.  And of course, we’ll administer medicines and wellness checks, and care for toileting, ambulation, bathing and eating as needed.

There is even a monthly support group for you, the caregiver.
